🩺 Role Overview: Diabetes CNC
As a Diabetes CNC, you will play a pivotal role in supporting patients with complex needs. Your responsibilities will include:
- Providing self-management education to patients living with diabetes
- Supporting patients and team members in multidisciplinary diabetes and nephrology clinics
- Assisting patients undergoing dialysis to manage their changing insulin needs
- Collaborating with inpatient diabetes CNCs to ensure holistic care

- Be credentialed or working toward credentialing
- Demonstrate a commitment to lifelong learning

- Salary: Clinical Nurse Consultant Grade 4B
